目的:应用Cirrus HD-OCT 5000对正常眼黄斑及视盘血流参数进行血管成像测量,评估其可重复性及再现性。方法:纳入40只正常眼进行前瞻性研究。操作者A于一周内3天的同一时段(T1,T2,T3)对正常受试者的同一眼黄斑及视盘进行3次血管成像扫描,扫描过程中均开启FastTracTM图像跟踪功能,操作者B在T 2时间点再次对受试者同一眼进行相同程序扫描,使用Angio PlexMetrixTM量化软件(版本10.0)自动测量黄斑和视盘的血管长度密度(vessel length density,VD)和血管灌注密度(vascular perfusion density,PD)。应用单因素方差分析或非参数检验比较3次扫描的VD、PD是否有差异。采用组内相关系数(intraclass correlation coefficient,ICC)、变异系数(coefficient of variation,CV)评价操作者A的可重复性,采用一致性相关系数(consistent correlation coefficient,CCC)、重复性系数(repeatability coefficient,CR)、C V评价操作者间的再现性。结果:操作者A对黄斑及视盘各区域3次扫描的V D、P D之间差异无统计学意义(P >0.05)。操作者A对黄斑和视盘V D、PD 3次扫描的ICC值分别为0.260~0.517、0.362~0.898,黄斑及视盘各区域V D、P D的C V值均<8.1%。黄斑和视盘V D、P D的操作者间CCC值分别为0.3130~0.5665、0.5149~0.7801;黄斑VD和PD的CR值分别为3.2212~4.6399、0.0574~0.0832;视盘VD和PD的CR值分别为2.0675~4.0630、0.0447~0.0730。黄斑CV值均<9.0%,视盘CV值均<6.9%。结论:非同日的同一时段视盘浅层血流参数具有较好的重复性及再现性,黄斑浅层血流参数的重复性及再现性相对较差。
Objective: To evaluate the repeatability and reproducibility of quantification of macular and peri-papillary superficial capillaries in normal eyes using Cirrus HD-OCT 5000 angiography. Methods: Forty normal eyes were included in this prospective study. Angiography 3 mm × 3 mm scan protocol centered on the disc or macula of the same eye was repeated 3 times by operator A in the same period on 3 days (T1, T2, T3) within 1 week,and the FastTracTM image tracking function was turned on during the scan. Operator B repeated the same scan independently at T2 after operator A. Angio Plex MetrixTM quantification software (version 10.0) was used to automatically measure the vessel length density (VD) and vascular perfusion density (PD) of macular and peri-papillary superficial capillaries. One-way analysis of variance or nonparametric test was used to compare the VD and PD measurements of the 3 scans. The intraclass correlation coefficient (ICC) and coefficient of variation (CV)were calculated to evaluate the intra-operator repeatability. Consistent correlation coefficient (CCC), repeatability coefficient (CR), and CV were calculated to evaluate the inter-operator reproducibility. Results: There was no statistically significant difference in the measurements of VD and PD between the 3 scans of the macular and peri-papillary superficial capillaries by operator A (P>0.05). The ICC values for the 3 scans of VD and PD of the macular and peri-papillary superficial capillaries by operator A were 0.260–0.517 and 0.362~0.898, respectively.The CV values of the VD and PD of the macular and peri-papillary superficial capillaries were all <8.1%. The inter-operator CCC values of the macular and peri-papillary superficial capillaries were 0.3130–0.5665 and 0.5149~0.7801,respectively; the CR values of the VD and PD of macula were 3.2212–4.6399 and 0.0574–0.0832, respectively; the CR values of the VD and PD of optic disc were 2.0675–4.0630 and 0.0447–0.0730, respectively. The CV values of macular were all <9.0%, and the CV values of optic disc were all <6.9%. Conclusion:The macular and peri-papillary superficial capillaries parameters in the same period on different days have a good repeatability and reproducibility,while the superficial macular blood flow parameters have relatively poor repeatability and reproducibility.
目的:探讨以岗位需求为导向的培训模式在眼科手术室护士上岗前消毒供应专科专项培训中的应用效果。方法:手术室上岗前护士共44名,按随机分配原则,其中23名为实验组,另外21名为对照组。实验组以医院消毒供应中心三大行业标准和医院消毒技术规范为理论依据,以手术室护理岗位需求为目标,制定针对性培训内容,采用多方式培训方法,在消毒供应中心培训3个月考核合格后调入手术室。对照组直接调入手术室工作。采用手术室带教老师对新上岗护士满意度调查作为评价指标。结果:23名学员的理论知识考核、技术操作考核全部合格,小讲课顺利完成。手术室带教老师对实验组的满意度明显高于对照组。结论:建立以岗位需求为导向的眼科手术室护士消毒供应专科培训模式,有助于提高新上岗的手术室护士工作适应性,并提升手术室护士对专科器械的管理能力。
Objective: To explore the application effect of job demand-oriented training model in specialized training of sterile supplying for nurses in the ophthalmic operation room. Methods: A total of 44 nurses who would work in the operating room were randomly divided into the experimental group (n=23) and control group (n=21). Based on the three industrial standards of Central Sterile Supply Department (CSSD) and the technical specifications of disinfection in hospital, and targeting the job requirements of operating room nursing, the experimental group formulates specific training contents and adopts various training methods. The experimental group was transferred to the operating room after being trained in CSSD for 3 months and passing the examination. The control group was transferred directly to the operating room. The degree of satisfaction from the teachers of operating room to new nurses was used as the evaluation index. Results: All the 23 students passed the examination of theoretical knowledge and technical operation, and the small lectures were completed successfully. The degree of satisfaction from teachers in operating room to trainees was obviously higher than that of untrained nurses. Conclusion: The establishment of job demand-oriented training model in specialized training of sterile supplying for nurses in ophthalmic operation room is helpful to improve the work adaptability of the newly-hired nurses in the operation room. It also helps the nurses in the operation room to enhance the management ability on the specialized instruments.
目的:探讨品管圈在提高玻璃体切除术后患者俯卧位执行率中的应用效果。方法:成立品管圈活动小组,选定提高玻璃体切除术后患者俯卧位执行率作为活动主题,运用品管圈工具和方法,对现状进行分析,找出影响因素,制定相应的措施并组织实施,比较品管圈活动前后玻璃体切除术后患者俯卧位执行率情况。结果:玻璃体切除术后患者俯卧位执行率由活动前的64.70%提高到95.83%。结论:运用品管圈能显著提高玻璃体切除术后患者俯卧位执行率,值得临床推广。
Objective: To explore the effect of quality control circle on improving the prone position execution rate of patients after vitrectomy. Methods: Establish quality control circle activity group, select improving the prone position execution rate of patients after vitrectomy as the activity theme, use quality control circle tools and methods to analyze the status quo, find out the influencing factors, formulate corresponding measures and implement them, and compare the prone position execution rate of patients before and after quality control circle activity. Results: The prone position execution rate of patients after vitrectomy increased from 64.70% to 95.83%.Conclusion: The application of quality control circle can significantly improve the prone position implementation rate of patients after vitrectomy, which is worthy of clinical promotion.
目的:探讨急救护理技能规范化培训考核在眼科护士中的应用效果。方法:对暨南大学附属深圳眼科医院142名眼科护士实施急救护理技能的规范化培训考核,比较培训前后急救理论考核成绩及急救技能操作考核成绩的差异,并采用自制调查问卷进行培训评价的调查。结果:在实施急救护理技能规范化培训后,眼科护士的急救理论与技能操作考核成绩均高于培训前,差异有统计学意义(P<0.05);90%以上的眼科护士对急救培训持肯定意见。结论:急救护理技能规范化培训考核提高了眼科护士的临床急救护理水平,培养了良好的急救护理意识和团队协作精神,同时增加了护士的自我价值及职业认同感,获得了较好的培训效果。
Objective: To explore the application effect of standardized training and assessment of emergency nursing skills in ophthalmology nurses. Methods: We implemented standardized training and assessment of emergency nursing skills for 142 ophthalmology nurses in our hospital, compared the difference between the first aid theory and the first aid skill operation before and after the training, and used a self-made questionnaire to conduct training evaluation surveys. Results: After implementing the standardized training of emergency nursing skills, the assessment scores of first aid theory and skill operation of ophthalmology nurses were higher than those of the pre-training assessment. And the difference was statistically significant (P<0.05). More than 90% of ophthalmology nurses had a positive opinion on first aid training. Conclusion: Standardized training and assessment of emergency nursing skills have improved the clinical emergency nursing level of ophthalmology nurses and cultivated a good emergency nursing awareness and teamwork spirit. At the same time, they have strengthened nurses’ self-worth and professional identity, and achieved better training results.

Due to its unique self-renewal and multi-directional differentiation capabilities, mesenchymal stem cells (MSCs) have become an emerging therapy that replaces of ineffective conventional options in treating multiple diseases. It plays an important role in ocular surface illnesses through a variety of functions, including immunoregulation, promoting repairing, anti-angiogenesis mechanisms, and cell replacement. MSCs from different sources have different mechanisms.So far, 8 registered clinical trials of MSCs therapy have been applied to treat dry eye, ocular burn, keratoconus and other ocular surface diseases.

Dry eye is one of the most common ocular surface diseases. It is characterized by a loss of homeostasis of the tear film, and accompanied by ocular symptoms, in which tear film instability and tear hyperosmolarity, ocular surface inflammation, and neurosensory abnormalities play major etiological roles. Diquafosol tetrasodium is a purinergic P2Y2 receptor agonist that promotes mucin and aqueous tear secretion. The unique pharmacological mechanism of diquafosol tetrasodium opens up a new direction for the medical therapies of dry eye. This article reviews the clinical therapeutic effect and research progress of diquafosol tetrasodium for the past few years.

Topical administration is the most common method of ocular medication, but it is generally difficult for the drug to pass through the cornea, and the bioavailability of the drug is low. Nanocarrier drugs were used in eyes in the 1980s, and liposomes and lipoids vesicles (Niosomes) interacted with ocular surface mucins to prolong the residence time of the drug on the ocular surface. Nanoemulsion surfactants can release the tight junctions of corneal epithelial cells, form transport openings, and inhibit the degradation of pharmaceutically active proteins by cell surface glycoprotein P (Pgp). Nanoparticles pass through the corneal and conjunctival epithelium without causing toxicity. Nanocapsules internalize deeper into the corneal epithelium (at 50 μm). Polymeric micelles self-assemble into core-shell nanocarriers to enhance the ability of drugs to penetrate the cornea. Anionic high-generation poly-amidoamine (PAMAM) dendrimers enhance drug permeability. Neutral and cationic low-generation dendrimers mediate higher drug permeability through clathrin pathway. Nanocrystal, in addition to enhancing drug solubility and dissolution rate, its high adhesion ability helps drug retention and penetration into ocular tissues. Nanostructured materials are closely related to dry eye and provide a choice for the treatment and diagnosis of dry eye.
近视是危害儿童青少年视力最常见的眼部疾病,高度近视对视功能造成极大的威胁。近年来,我国近视发病率逐年升高,对近视筛查与防控的需求也不断增加,随着人工智能理论与技术的不断发展与成熟,可以辅助眼科医生进行近视筛查、诊断与治疗。本文将简要介绍人工智能在近视的筛查、预测、检测、病理性近视以及角膜屈光手术中的应用,浅谈了目前人工智能在研究中存在的可比度较低、影像要求较高、可解释性较低及隐私保护等问题,并展望人工智能在近视相关领域的应用前景。
Myopia is the most common ocular disease that harms the vision of children and adolescents. High myopia poses a great threat to visual function. The incidence of myopia in China has been increasing in recent years, and the demand for myopia screening, prevention and control has also expanded. With the continuous development of artificial intelligence theory and technology, Artificial intelligence can assist ophthalmologists in myopia screening, diagnosis and treatment. This review will briefly introduce artificial intelligence in the screening, prediction, and detection of myopia; also, the application in pathological myopia and corneal refractive surgery. This review will discuss some problems of current artificial intelligence research, such as low comparability, high image requirements, low interpretability, privacy protection, and the application prospects of artificial intelligence in myopia.
目的:探讨品管圈活动提升眼科带螺母器械清洗质量的效果。方法:中山大学中山眼科中心珠江新城院区自2020年8月1日到2020年9月3 0日消毒供应中心处理的眼科带螺母器械(以量规为例)共2 076件,8月31日前为对照组,按常规清洗流程处理;9月1日后为实验组,成立品管圈小组,针对器械结构制定相应清洗流程。结果:实验组的清洗合格率为98.7%,对照组的清洗合格率为93.4%,差异有统计学意义(P<0.01)。结论:实施品管圈活动可提升眼科带螺母器械的清洗质量。
Objective: To explore the effect of quality control circle activity on improving the cleaning quality of ophthalmic equipment with nuts. Methods: The object of the study was 2 076 pieces of ophthalmic equipment with nuts (taking gauge as an example) used from August 1, 2020 to September 30, 2020 in the central sterile supply department of the Zhongshan Ophthalmic Center of Sun Yat-sen University in Zhujiang New Town. The equipment used before August 31 was set as the control group and was dealt with common cleaning process. The equipment used after September 1 was set as experimental group and the quality control circle group was set up to develop corresponding cleaning procedures for the instrument structure. Results: The qualified rate of cleaning in the experimental group was 98.7%, and that in the control group was 93.4%. The difference was statistically significant (P<0.01). Conclusion: Quality control circle activity can improve the cleaning quality of ophthalmic equipment with nuts.
目的:探讨PDCA循环管理模式在优化白内障日间患者诊疗流程中的应用与成效。方法:选取中山大学中山眼科中心白内障病区2018年9月至2019年12月收治的白内障日间患者400例作为试验对象,选择2018年9月至2019年4月200例患者作为优化前组,选择实施PDCA循环管理模式后的2019年5月至2019年12月的200例患者作为优化后组。统计并比较两组患者实施前后的术前检查时长、住院时长、术后第1天复诊路径及患者满意度。结果:PDCA循环管理法实施了7个月后,患者术前检查时长由优化前的(2.94±2.12) h降至(2.09±0.93) h,住院时长由优化前的(22.73±1.14) h 降至(5.22±1.29) h,差异均具有统计学意义( P <0.001);患者满意度由优化前的92%上升至96%(P<0.05);术后第1天,患者复查所需行走的路径缩短,由以前的172 m降至129 m。结论:实施PDCA循环管理模式能有效地改善白内障日间患者的诊疗流程,缩短患者诊疗时间,提高患者的满意度,值得临床推广。
Objective: To explore the application and effect of Plan-Do-Check-Act (PDCA) cycle management mode in optimizing the diagnosis and treatment process of cataract patients with daytime surgery. Methods: A total of 400 cases of patients with daytime surgery admitted to Department of Cataract, Zhongshan Ophthalmic Center,Sun Yat-sen University from September 2018 to December 2019 were selected as the experimental subjects. 200 patients from September 2018 to April 2019 were selected as the pre-optimization group, and 200 patients from May 2019 to December 2019 after the implementation of the PDCA cycle management mode were selected as the post-optimization group. Time of preoperative examination, length of hospital stay, the follow-up path in the first day after surgery and patient satisfaction were statistically compared between the two groups before and after implementation. Results: Seven months after the implementation of PDCA cycle management mode, the preoperative examination time decreased from (2.94±2.12) h to (2.09±0.93) h, and the length of hospital stay decreased from (22.73±1.14) h to (5.22±1.29) h, and the differences were statistically significant (P<0.001); the patients’ satisfaction increased from 92% to 96% (P<0.05); the follow-up path in the first day after operation was changed from the 13th floor (172 m) to the 3rd floor (129 m). Conclusion: The implementation of PDCA cycle management mode can effectively improve the diagnosis and treatment process of cataract patients with daytime surgery, shorten the diagnosis and treatment time and improve the satisfaction of patients, so it is worthy of clinical promotion.
