息肉状脉络膜血管病变(polypoidal choroidal vasculopathy,PCV)是亚洲人群中常见的致盲性眼病,发生大出血并发症后严重危害视力且预后差。PCV大出血包括视网膜下出血(subretinal hemorrhage,SRH)和玻璃体积血(vitreous hemorrhage,VH)。SRH的危险因素包括较长病程、簇型PCV、息肉状病灶不消退、合并视网膜色素上皮脱离;其治疗方式包括抗血管内皮生长因子药物、光动力疗法、激光、玻璃体腔注气、眼内注射组织纤溶酶原激活剂、玻璃体切割术或联合治疗等方式,其中,黄斑中心凹是否受累和出血时间是影响治疗方式选择的主要因素。发病年龄较大、白细胞计数较高、天门氨酸转移酶和丙氨酸转氨酶的比值较高、活化部分凝血活酶时间较长、曾行光动力疗法、有玻璃体腔注药治疗史、SRH面积大、出现视网膜色素上皮脱离的PCV患者发生VH的风险高。浓厚的VH通常需行玻璃体切割术,其手术时机和手术方式的选择是临床关注的焦点。鉴于目前PCV大出血的危险因素尚不完全明确、治疗方面也尚未形成共识,需要开展相关临床研究,提供更多依据。
Polypoidal choroidal vasculopathy (PCV) is a common blinding disease in Asian populations. Massive hemorrhage complications secondary to PCV includes subretinal hemorrhage (SRH) and vitreous hemorrhage (VH). The risk factors for SRH include a long duration, clustered PCV, non-regression of polyp lesions and presented with retinal pigment epithelial detachment. The treatments for SRH including anti-vascular endothelial growth factor drugs, photodynamic therapy, laser, vitreous pneumatic displacement, intravenously injected tissue plasminogen activator, vitrectomy and combination therapy. Whether macular fovea is involved and the time since bleeding onset are the main factors afecting the choice of treatment for SRH. Older age of onset, higher white blood cell count, higher aspartate amino transferase and alanine amino transferase ratio, longer activated partial thromboplastin time retinal pigment epithelium detachment, photodynamic therapy history, intravitreal injection history larger SRH area and presented with retinal pigment epithelial detachment were associated with higher risk of VH. PCV patients with massive VH should be treated with vitrectomy, while the timing and technique of operation should be paid atention to. At present, the risk factors of PCV massive bleeding are not completedly clear, and its treatment methods are diverse, which requires a large number of studies to prove its efectiveness and establish expert diagnosis and treatment consensus.
肥厚型脉络膜谱系疾病(pachychoroid disease spectrum,PCD)包括肥厚型脉络膜色素上皮病变(pachychoroid pigment epitheliopathy,PPE)、中心性浆液性脉络膜视网膜病变(central serous chorioretinopathy,CSC)、肥厚型脉络膜新生血管病变(pachychoroid neovasculopathy,PNV)、息肉样脉络膜血管病变(polypoidal choroidal vasculopathy,PCV)、局灶性脉络膜凹陷(focal choroidal excavation,FCE)和盘周肥厚型脉络膜综合征(peripapillary pachychoroid syndrome,PPS)。有学者将PCD看作脉络膜功能障碍引发的一系列连续疾病过程,但关于PCD的发病机制、形态改变尚未明确。该文对PCD的脉络膜、涡静脉及巩膜相关改变做一综述。
Pachychoroid disease spectrum include pachychoroid pigment epitheliopathy, central serous chorioretinopathy, pachychoroid neovasculopathy, polypoidal choroidal vasculopathy, focal choroidal excavation, and peripapillary pachychoroid syndrome. Currently, some scholars regard pachychoroid disease spectrum as a series of continuous disease processes caused by choroidal dysfunction, but the pathogenesis and morphological changes of pachychoroid disease spectrum are not yet clear. This paper reviews the changes of choroid, vortex veins and sclera in pachychoroid disease spectrum.
目的:评估远视儿童使用1%阿托品凝胶1周后脉络膜厚度(choroidal thickness,CT)的变化。方法:选择42例4~7岁的远视儿童,予每天使用1%阿托品凝胶两次,持续7d。使用光学相干断层成像扫描测量视网膜及CT,并分析使用1%阿托品凝胶前后中心凹以及距中心凹处间隔1.0 mm的上、下、鼻和颞侧(最多3.0mm)CT的变化。结果:在远视儿童中,基线CT随位置而变化(F=27.08, P<0.05),与中心凹相比,鼻侧及距中心凹上方2 mm、3 mm及距中心凹颞侧3 mm处的CT较薄(P<0.05)。使用1%阿托品凝胶后,中央凹及旁中心凹CT改变比较差异无统计学意义(P>0.05)。使用1%阿托品凝胶前后视网膜厚度无明显变化(P>0.05)。结论:短期使用1%阿托品凝胶并没有改变远视儿童的脉络膜和视网膜厚度。
Objective: To assess changes of choroidal thickness (CT) in hyperopia children after 1 week using of 1% atropine.Methods: A total of 42 hyperopia children aged 4–7 years were included into the study.A single drop of 1% atropinegel was used twice a daily for 7 days in the subjects.The thickness of retina and choroid was measured by OCT, and the changes before and after using 1% atropine gel were analyzed at the subfovea and at 1.0 mm intervals (up to3.0 mm) from the fovea at superior, inferior, nasal, and temporal locations. Results: In the hyperopia children, baselineCT parameters were varied with the location(F=27.08,P<0.05).Compared with the fovea, the CT at the nasal side,2 mm and 3 mm above the fovea and 3 mm from the temporal side of the fovea were thinner (P<0.05).After using 1%atropine gel, there was no significant difference in the CT changes of subfoveal choroidal thickness and other sites ofparafovea (P> 0.05). There was no significant change in retinal thickness before and after using 1% atropine gel (P > 0.05).Conclusion: No changes were found in the thickness of choroid and retina in hyperopia children after short-term use of1% atropine gel.
息肉样脉络膜血管病变(polypoidal choroidal vasculopathy,PCV)是一种以脉络膜异常分支血管网和血管末端息肉状脉络膜血管扩张灶为特征的眼底疾病。(indocyanine green angiography,ICGA)是诊断PCV的金标准,可见异常分支血管网、息肉样高荧光。(optical coherence tomography,OCT)可见色素上皮脱离(pigment epithelial detachments,PED)、双层征、指状突起、PED切迹、脉络膜增厚等。基于 OCT 的影像学研究表明,PCV属于肥厚型脉络膜谱系疾病(pachychoroid disease spectrum,PCD),其特征是脉络膜增厚、脉络膜大血管层(Haller层)扩张,伴有毛细血管层(Ruysch层)和中血管层(Sattler层)变薄,其发病机制可能涉及脉络膜、涡静脉、巩膜等相关改变。
局限性脉络膜凹陷(focal choroidal excavation,FCE)是表现为光感受器细胞层、视网膜色素上皮层向脉络膜层凹陷的眼底病变。发病机制多认为是先天脉络膜发育异常、炎症或感染。基于光学相干断层扫描(optical coherence tomography,OCT),可将FCE从光感受器尖端与视网膜色素上皮层是否分离、形态特征或中央脉络膜厚度三种方式对其进行分类。FCE常合并脉络膜新生血管等疾病。该文对FCE的临床研究现状做一综述。
Focal choroidal excavation (FCE) is a retinopathy, expressing with a depression in photoreceptor cell layer and retinal pigment layer towards the choroidal layer. The pathogenesis is recongized as congenital choroidal developmental abnormalities, inflammation, or infection. Based on optical coherence tomography (OCT), FCE can be classified by three methods: the situation of photoreceptor separating from the retinal pigment upper cortex, morphological features, or central choroidal thickness. FCE is usually associated with other diseases such as choroidal neovascularization. This article reviews the current clinical research on FCE.
目的:比较水合氯醛和阿佛丁两种药物对小鼠的麻醉作用以及不同激光能量对脉络膜新生血管造模成功率的影响。方法:24只C57/BL6小鼠随机分为3组,分别予以200,300和400 mW的532 nm激光进行视网膜光凝;每组再按1:1分为2个亚组,随机使用4.3%水合氯醛或1.2%阿佛丁进行小鼠腹腔注射麻醉。在激光光凝后第4,7,1 0和1 4天进行视网膜荧光素造影(Fundus Fluorescein Angiography,FFA)、SD-OCT检查,在第1 4天时对脉络膜行IB4染色检查。结果:阿佛丁和水合氯醛诱导小鼠进入麻醉时间长分别为(120±30) s和(150±45) s,持续时间分别为(30±15) min和(50±20) min,差异均有统计学意义(均P=0.0001);发生不可逆性白内障的比率分别为2/24和1/24,麻醉导致的死亡数分别为1和0只,差异均无统计学意义(P =0.551,P =0.300)。200,300和400 mW三个激光能量组的造模成功率分别为9.38%,37.5%和93.75%,差异有统计学意义(P=0.0001)。FFA显示:造模成功的小鼠在4 d时荧光素渗漏达到高峰,随后荧光素溢漏的面积逐渐减小。结论:与阿佛丁相比,水合氯醛对小鼠的麻醉维持时间长且不会增加病死率和不可逆性白内障的发生率;400 mW的532 nm激光能量是诱发CNV的较好设置。
Objective: To compare the narcotic effects of chloral hydrate and avertin in mouse models and the success rate of choroidal neovascularization (CNV) mouse model establishment with different energy hierarchy of laser. Methods: Twenty-four mice were randomized into 3 groups, the 532 nm laser was used with different power levels of 200, 300 and 400 mW for each group. In each group, the chloral hydrate was used for 4 mice and avertin for the other 4 mice at random. On day 4, 7, 10 and 14, all mice were tested by the fundus fluorescein angiography (FFA), SD-OCT, and the choroid was stained with the isolectin B4 conjugated AF488 on day 14. Results: The anesthesia induction time of avertin and chloral hydrate was (120±30) s and (150±45) s, and the duration of anesthesia was (30±15) min and (50±20) min, the differences were statistically significant (all P=0.0001); the incidence of irreversible cataract of avertin models and chloral hydrate models was 2/24 and 1/24, the number of death in two models was 1 and without statistical significance (P=0.551, P=0.300). With the increase of laser energy, the success rate of experiments was also gradually increased, from 9.38%, 37.5% to 93.75% (P=0.0001) in 200, 300 and 400 mW group, respectively. According to the results of FFA, the peak of fluorescence leaking was at day 4 after the laser burning, and was gradually reduced. Conclusion: Compared with avertin chloral hydrate can improve the duration of anesthesia without increasing the risk of cataract and mortality. As for the 532 nm laser, the 400 mW energy could be an optimal parameter for the laser-induced CNV.
眼结核的表现形式多样,有时会被误诊漏诊。本病例为一年轻男性,以右眼视力突发下降为主诉就诊,眼部表现可见黄斑区脉络膜占位病灶及视网膜内小病灶,经γ-干扰素释放试验、胸部CT等检查,确诊为血型播散性肺结核,眼部诊断为结核性脉络膜结节联合视网膜结节,全身接受抗结核治疗后,眼部病灶消退。
Ocular tuberculosis encompasses a variety of clinical manifestations, which is easily misdiagnosed sometimes. In this article, a young male was admitted to our hospital with a chief complaint of sudden blurred vision in his right eye. His ocular examination detected a tiny mass in the choroid and a small lesion in the retina. After interferon-gamma release assay (IGRA) and chest CT scan, the patient was diagnosed with military tuberculosis. The ocular examination confirmed the diagnosis of choroidal tubercle complicated with intrarentinal tubercle, which were healed after systemic anti-tuberculosis therapy.
脉络膜破裂大部分是由撞击引起的对冲伤,冲击力通过玻璃体传导眼底引起,因此一般脉络膜破裂会有比较明确的外伤或者钝挫伤病史。本文将报告1例16岁体校男生在无明显外伤史出现多发性脉络膜破裂伤,通过查阅文献发现有文献报道在隐匿性假性黄色瘤(pseudoxanthoma elasticum,PXE)疾病中可在无明显外伤或轻微外伤出现脉络膜破裂,并根据文献复习考虑本病例为隐匿性PXE可能。
Most of the choroidal rupture is mostly caused by impact injury, and the impact force is caused by the vitreous conduction through the fundus. Therefore, the choroidal rupture generally has a clear history of trauma or blunt trauma. This article will report a case of a 16-year-old boy in a sports school who developed multiple choroidal ruptures without obvious trauma history. Through literature review, it was found that choroidal rupture can occur without obvious or minor trauma in subtle pseudoxanthoma elasticum (PXE) disease, and based on literature review, this case was considered as a possibility of subtle PXE.
报道1例人类免疫缺陷病毒(human immunodeficiency virus,HIV)眼部巨细胞病毒性视网膜炎(cytomegalovirus retinitis,CMVR)合并脉络膜结核瘤感染患者,主因双眼视物模糊2周就诊。经眼部检查发现右眼底颞侧视网膜广泛黄白色颗粒样病变,病灶边界可见黄白色奶酪样渗出,左眼下方视网膜大片黄白色渗出伴出血。在随访半年后发现左眼视网膜脉络膜隆起病灶,根据其全身及眼部临床特征,诊断为双眼CMVR伴左眼脉络膜结核瘤,予全身抗病毒及结核治疗后随访1年余,全身情况及眼部病灶稳定。
A case of human immunodeficiency virus (HIV) cytomegalovirus retinitis (CMVR) complicated with choroidal tuberculoma infection was reported. The patient visited hospital due to bilateral blurred vision for 2 weeks. Ocular examination showed extensive yellowish-white granular lesions in the temporal retina of the right fundus, with yellowish-white cheese-like exudation at the border of the lesion, and a large yellowish-white exudation with hemorrhage at the lower part of the left eye’s retina. After six months of follow-up, the patient was found to have a retinal choroid hump in the left eye. Based on her systemic and ocular clinical features, the patient was diagnosed as bilateral CMVR with choroidal tuberculoma of the left eye. The patient had her follow up check-up a year after her systemic antiviral and anti-tuberculosis treatment with her general condition stable and ocular lesions treated.
息肉状脉络膜血管病变(polypoidal choroidal vasculopathy,PCV)是亚洲人中常见的眼底致盲性疾病,当PCV合并视网膜下出血或玻璃体积血(vitreous hemorrhage,VH)时,患者视力骤然下降,视力预后差异大。但目前聚焦于PCV合并VH的相关文献较少,因此研究和阐明PCV继发VH的治疗方法及预后具有重要的临床意义。目前临床上常选择手术干预,玻璃体切除术(pars plana vitrectomy,PPV)是临床上最常选择的一种术式。其他治疗方式包括玻璃体内注射抗血管内皮生长因子(vascular endothelial growth factor,VEGF)、眼内气体或硅油填充、眼内注射组织纤溶酶原激活剂(tissue plasminogen activator,tPA)和光动力疗法(photodynamic therapy,PDT)。PCV合并VH患者的视力预后决定因素是黄斑视功能的保留程度,也与年龄、术前视力、PCV病变部位、视网膜下出血量、视网膜脱离范围、基线黄斑中心厚度(central macular thickness,CMT)、是否出现术后并发症以及是否形成视网膜瘢痕等因素相关,目前也有研究发现视力预后与单核苷酸多态性(single nucleotide polymorphisms,SNP)相关。本文就PCV继发VH的临床特点、治疗及预后进行综述。
Polypoid choroidal vasculopathy (PCV) is a common fundus blinding disease in Asians. When PCV is associated with subretinal hemorrhage or vitreous hemorrhage (VH), patient's visual acuity decreases suddenly and the visual prognosis varies greatly. There are few relevant literatures focusing on VH secondary to PCV, so it is of great clinical significance to study and clarify the treatment methods and prognosis of VH secondary to PCV. At present, surgical intervention is often selected in clinical practice. Vitrectomy is the most commonly selected surgical procedure in clinical practice. The other treatment modalities include intravitreal injection of antivascular endothelial growth factor (VEGF), intraocular gas or silicone oil filling, intraocular injection of tissue plasminogen activator (tPA) and photodynamic therapy. The prognostic determinant of visual acuity in PCVpatients with VH is the degree of preservation of macular visual function. The prognostic is also related to age, preoperative visual acuity, PCV lesion location, amount of subretinal hemorrhage, extent of retinal detachment, baseline central macular thickness (CMT), postoperative complications and retinal scars. Recent studies also find that the prognosis of visual acuity is related to single nucleotide polymorphisms. This article reviews the clinical characteristics, treatment and visual prognosis of PCV associated with VH.
