SMILE术后角膜体积的变化及其与角膜生物力学特性的相关性

Correlation of the changes in corneal volume with the corneal biomechanical parameters after small incision lenticule extraction

目的:基于新一代Corvis® ST分析成人近视患者经飞秒激光小切口角膜透镜取出术(small incision corneal lens extraction,SMILE)后角膜体(corneal volumn,CV)的变化与角膜生物力学变化的相关性,进一步证明C V对于评估SMILE术后角膜生物力学特性以及预测屈光手术结果的价值。方法:采用前瞻性病例观察研究方法,纳入2021年1月至8月在佛山市第二人民医院拟行SMILE的成人近视患者72例(72眼)。为避免双眼相关性,纳入患者的一个合适眼(右眼或者左眼),根据术前中央角膜厚度,将厚度470~<530 μm分为低角膜厚度组,530~<560 μm为中角膜厚度组 , ≥560μm为高角膜厚度组。应用新一代 Corvis® ST 于术前和术后3个月测量非接触生物力学校正眼压(biomechanical intraocular pressure,bIOP)、2 mm处变形幅度的比值[DA ratiomax(2 mm),DA2ratio]、综合半径(integrated inverse concave radius,IR)、水平方向Ambr?sio相关厚度,最薄点厚度/厚度变化率(Ambrósio’s relational thickness,ARTh)、第1次压平时角膜硬度参数(stiffness parameter applanation 1,SP-A1)、Corvis生物力学指数(Corvis biomechanical index,CBI)和应力和应变参数(stress-strain index,SSI)7项角膜生物力学参数。应用Pentacam眼前节分析系统测量中央角膜厚度(central corneal thickness,CCT)和不同区域的CV(CV3mm、CV5mm、CV7mm和CV10mm)。对比SMILE术前与术后3个月的CV、角膜生物力学参数的变化,并分析角膜生物力学参数与CV的相关性。数据采用配对、独立样本t检验分析差异性;单因素方差分析不同角膜厚度组的差异性;Spearman相关性分析和偏最小二乘回归(partial least squares linear regression,PLSLR)分析相关性。结果:SMILE术后3个月,CV3mm、CV5mm、CV7mm和CV10mm均较术前显著下降(均P<0.001);bIOP、ARTh、SP-A1和SSI较术前明显下降(P<0.05);DA2ratio、IR和CBI较术前明显升高(P<0.05)。术前和术后3个月不同角膜厚度组的ARTh、SP-A1DA2ratio、IR和CBI差异均有统计学意义(均P<0.05),低中角膜厚度组SSI术前后变化不大(P>0.05)。术前DA2ratio与CV3mm、CV5mm呈负相关,与CV10mm呈正相关;ARTh与CV3mm、CV5mm呈正相关,与CV10mm呈负相关;bIOP与CV10mm呈正相关;IR与CV3mm、CV5mm呈负相关,与CV10mm呈正相关;SP-A1与CV3mm、CV5mm呈正相关,与CV10mm呈负相关;CBI与CV3mm、CV5mm和CV10mm呈负相关,SSI与CV3mm呈正相关,与CV10mm呈负相关。SMILE术后3个月,ARTh与CV3mm、CV5mm呈正相关,与CV10mm呈负相关;CBI与CV3mm负相关、CV10mm呈正相关(均P<0.05)。SMILE术后3个月变化量中,ARTh与CV3mm、CV5mm呈正相关,与CV10mm负相关;CBI与CV3mm呈负相关,与CV10mm呈正相关(均P<0.05)。结论:成人SMILE术后3个月CV3mm、CV5mm、CV7mm、CV10mm、眼压和CCT较术前均显著降低;越往周边区域CV减少量越多。SMILE术后3个月的新一代Corvis® ST角膜生物力学新型参数较术前有显著变化;不同区域CV与角膜生物力学参数存在一定的相关性,预期SMILE术后不同区域的CV可以表征术后部分角膜生物力学特性的改变,CV可能成为表征预测角膜扩张发展的参数因素。
Objective: To evaluate the changes in corneal volume (CV) and the correlation of corneal biomechanics in adult myopic patients after small incision corneal lens extraction (SMILE) based on the new generation of Corvis® ST.This paper further demonstrates the value of CV in evaluating corneal biomechanical properties after SMILE and predicting the outcome of refractive surgery. Methods: A total of 72 cases (72 eyes) of adult myopia who were scheduled for SMILE in The Second People’s Hospital of Foshan from January to August 2021 were included in this prospective observational case study. To avoid binocular correlation, a suitable eye for each person was selected for inclusion. Based on the preoperative central corneal thickness (CCT), the cases were divided into a low corneal thickness group (470–<530 μm), a medium corneal thickness group (530–<560 μm), and a high corneal thickness group (≥560 μm). The biomechanical intraocular pressure (bIOP), DA ratiomax (2 mm) (DA2ratio), integrated inverse concave radius (IR), Ambrósio’s relational thickness (ARTh), stiffness parameter applanation 1 (SP-A1),corvis biomechanical index (CBI) and stress-strain index (SSI) were measured by Corvis® ST. The Pentacam anterior segment analysis system was used to measure partial CV and total CV with a diameter of 3, 5, and 7 mm from the top of the cornea, with an assumed diameter of 10 mm. The changes in corneal biomechanical parameters and CV before SMILE and 3 months after SMILE were compared. The correlation between corneal biomechanical parameters and CV was studied. Data were analyzed by paired and independent sample t-tests. One-way analysis of variance (ANOVA) was used to assess the differences between the corneal thickness groups. Spearman correlation analysis and partial least squares linear regression (PLSLR) were used for correlation analyses. Results: Three months after SMILE, the CV in different areas (CV3mm, CV5mm, CV7mm, and CV10mm) were significantly lower than those before SMILE, and the differences were statistically significant (all P<0.001). The corneal biomechanical parameters bIOP, ARTh, SP-A1, and SSI were significantly decreased compared with those before the operation (P<0.05). In contrast, the DA2 ratio, IR, and CBI were significantly higher than those before the operation (P<0.05). A comparison of the corneal biomechanical parameters ARTh, SP-A1, DA2 ratio, IR, and CBI in the different corneal thickness groups before and after SMILE showed overall differences among the groups (P<0.05).However, there was no significant change in the SSI between the low and medium corneal thickness groups. The correlation of the preoperative baseline DA2ratio was negatively correlated with CV3mm and CV5mm, and positively correlated with CV10mm; Conversely, ARTh was positively correlated with CV3mm and CV5mm, and negatively correlated with CV10mm; BIOP was positively correlated with CV10mm; IR was negatively correlated with CV3mm and CV5mm, and positively correlated with CV10mm; SP-A1 was positively correlated with CV3mm and CV5mm, and negatively correlated with CV10mm; CBI was negatively correlated with CV3mm, CV5mm, and CV10mm, while SSI was positively correlated with CV3mm and negatively correlated with CV10mm (all P<0.05). At 3 months after SMILE, ARTh was positively correlated with CV3mm and CV5mm, and negatively correlated with CV10mm; CBI was negatively correlated with CV3mm, and positively correlated with CV10mm (all P<0.05). Conclusion: This study demonstrated that at 3 months after SMILE, CV3mm, CV5mm, CV7mm, CV10mm, intraocular pressure, and CCT in the central and peripheral areas decreased significantly compared with to those before the operation, and the more peripheral the area, the more the CV decreased. Three months after SMILE, the new corneal biomechanical parameters of the new generation Corvis® ST changed significantly compared to those before the operation, and there was a certain correlation between CV in different regions and corneal biomechanical parameters. It is expected that CV in different regions after SMILE can characterize some changes in corneal biomechanical properties after SMILE, and CV may become a parameter factor for characterizing and predicting the development of corneal dilatation.

有晶体眼人工晶状体植入术后视疲劳症状及影响因素

Outcomes of asthenopia after implantable collamer lens implantation and its related factors

目的:评估屈光不正患者有晶体眼后房型人工晶体(implantable collamer lens,ICL)植入术后视疲劳症状及调节集合功能、眼表、像差的变化,并探讨其对视疲劳症状的影响。方法:前瞻性病例观察分析。连续收集在沧州市中心医院行ICL手术并完成3个月随访的患者,测定术前、术后1周、1个月、3个月时的视疲劳评分、调节幅度(amplitude of accommodation,AA)、正相对调节和负相对调节(positive/negative relative accommodation,PRA/NRA)、调节灵敏度(accommodative facility,AF)、调节性集合(accommodative convergence,AC)与调节(accommodation,A)比率(AC/A),Schirmer实验、非侵入性泪膜破裂时间(noninvasive breakup time,NBUT)及高阶像差(higher order aberration,HOA),进行统计学分析。结果:ICL术后第1周视疲劳症状较术前明显加重,随时间推移逐渐减轻,术后1个月仍高于术前,术后3个月时恢复。AA术后1周时较术前降低,术后1个月、3个月时明显高于术前;AF术后1周时较术前下降,术后1个月比术前水平稍好,3个月时明显高于术前;PRA、NRA无明显变化;AC/A术后1周时较术前下降,术后1个月回复到术前水平,术后3个月较术前提高。术后1周、1个月及3个月的NBUT值均较术前明显下降,术后1周时最低;Schiermer值术后1周时轻度下降,术后1个月、3个月时基本恢复。术后的总HOA均较术前有所增加,但各个时间点之间无明显变化。相关性分析显示ICL术后AF越差、NBUT越低,视疲劳症状越重。结论:ICL术后视疲劳症状一过性加重,AF和NBUT是影响视疲劳变化的重要因素。
Objective: To evaluate the changes of visual fatigue symptoms, accommodative functions, ocular surface conditions, and high-order aberrations (HOA) after implantation of implantable collamer lens (ICL), and to explore their effects on asthenopia. Methods: It was a prospective observational case series. Patients with ametropia who underwent ICL surgeries and completed 3-month follow-up in our hospital were enrolled.Asthenopia scores, amplitude of accommodation (AA), positive/negative relative accommodation (PRA/NRA),accommodative facility (AF), the ratio of accommodative convergence and accommodation (AC/A), Schirmer test, non-invasive breakup time (NBUT), and HOA were examined before surgeries and at 1 week, 1 month and 3 months after surgeries, then statistically analyzed. Results: Symptoms of asthenopia were significantly worse at 1 week after ICL surgeries than those before surgeries, but increased gradually as time went by, eventually recovered at 3 months postoperatively. Among regulatory indicators, AA decreased 1 week postoperatively, but was significantly higher at 1 and 3 months after surgeries; AF was lower 1 week after surgery than baseline, slightly better at 1 month postoperatively, and significantly higher at 3 months postoperatively; PRA and NRA had no significant change; AC/A decreased 1 week after surgeries, returned to the baseline at 1 month postoperatively, and increased 3 months postoperatively. Tears and meibomian gland function index: NBUT values at 1 week, 1 month and 3 months after surgeries were significantly decreased compared with those before surgeries, and NBUT at 1 week postoperatively was the lowest; Schiermer values had a slight decrease at 1 week after surgeries, and basically recovered at 1 and 3 months after surgeries. HOA after surgeries were increased compared with those before surgeries, but there was no significant change between each time point. Correlation analysis showed that the lower AF and NBUT after ICL surgeries, the more severe the asthenopia symptoms. Conclusion: The symptoms of asthenopia aggravated transiently after ICL implantation surgeries, but improved gradually with time. AF and NBUT were important factors affecting the changes of asthenopia.

眼科住院医师规范化培训学员沟通技能态度与人际沟通能力的现状和相关性

Current status and correlation between attitudes and competency of interpersonal communication in ophthalmology standardized training trainees

目的:了解眼科住院医师规范化培训学员(规培生)的沟通技能态度和人际沟通能力现状。法:对中山大学中山眼科中心三个年级规培生的沟通态度和人际沟通能力进行问卷调查,并进一步分析其沟通态度与沟通能力的相关性。结果:共纳入196名规培生,沟通积极态度量表总分为51.38±6.52,消极态度为36.80±5.70,人际沟通能力为2.41±0.40,辅导与咨询维度得分为2.50±0.64,提供有效的负面反馈维度得分为2.61±0.50,支持性沟通维度得分为2.29±0.44。沟通技能的积极态度与人际沟通能力和三维度之间显著相关(P<0.01)。人际沟通技能得分与性别、规培年级、喜欢眼科专业的程度、得到家庭的关爱程度、得到朋友的关爱程度等因素显著相关(P<0.05)。结论:眼科规培生对沟通技能的积极态度优秀,但是总体沟通能力较弱,并受到多种因素影响。应充分重视规培生人际沟通能力的培养,开展医患沟通的专业化培训。
Objective: To evaluate the current status and correlations between attitudes and competency for interpersonal communication in ophthalmology standardized training trainee. Methods: A questionnaire survey was conducted on the communication attitudes and interpersonal communication ability of standardized training trainees from three grades in Zhongshan Ophthalmic Center, affiliated to Sun Yat-sen University, and the correlation between communication attitude and communication ability was further analyzed. Results: A total of 196 trainees were included. The scores of positive attitude, negative attitude, and interpersonal communication skills were 51.38±6.52, 36.80±5.70, and 2.41±0.40, respectively. In addition, the dimension of coaching and counseling scores were 2.50±0.64, the dimension of providing effective and negative feedback scores were 2.61±0.50, and the dimension of supportive communication scores were 2.29±0.44. The positive attitudes of communication skills were significantly related to the interpersonal communication skills and three-dimensionality (P<0.01). The interpersonal communication skills were significantly associated with sex, grades, interest in ophthalmology, social support from the family and friends (P<0.05). Conclusion: Ophthalmology trainees have excellent positive attitudes toward communication skills, but the overall communication skills are weak and influenced by a variety of factors. More attention should be paid to the development of interpersonal communication skills of the trainees, and specialized training in doctor-patient communication is warranted.

中山大学中山眼科中心研究生分子医学实验技能培训课程课前调查报告

A pre-course student survey report on the Postgraduate Molecular Medicine Laboratory Skills Training Course in the Zhongshan Ophthalmic Center of Sun Yat-sen University

目的:调查中山大学中山眼科中心参加分子医学实验技能培训课程的研究生学员上课前的实验基础,为研究生实验技术课程的备课提供参考。方法:对2020年及2021年参加分子医学实验技能培训课程的博士和硕士研究生进行课前问卷调查。从理论知识和操作经验两方面对15个专题课程进行分析。结果:共155名学生完成问卷调查。在全部专业研究生中,25%以上学生对理论知识不了解的专题有14个,而仅有10%以下学生熟练操作的专题有6个;分子医学专业的优于前者,不了解和熟练掌握的专题个数分别是9和12。理论掌握程度最低的4个专题是细胞增殖、流式细胞、图像采集和组学分析。15个小专题整合成7个大专题,操作熟练程度由低到高依次是:眼科实验数据收集、组学技术、数据分析、细胞技术、蛋白技术、组织分析、分子技术。结论:新入学研究生存在分子医学实验技能基础理论知识薄弱以及科研训练不足等问题,迫切需要改善实验技能培训制度,深化课程体系建设。
Objective: To conduct a pre-course survey on the graduate students who participated in the Molecular Medicine Laboratory Skills Training Course in the Zhongshan Ophthalmic Center of Sun Yat-sen University to provide a reference for the preparation of the course. Methods: We asked the doctoral and master’s students who participated in the training courses of molecular medical experimental skills in 2020 and 2021 to take a pre-course questionnaire survey and analyzed the results by the students’ pre-course command of theoretical knowledge and pre-training operational experience in 15 lessons of special topics or techniques. Results: A total of 155 students completed the pre-course questionnaire survey. Our results showed that there were 14 lessons in which more than 25% of the students were not familiar with the theoretical knowledge, whereas there were 6 lessons in which less than 10% of the students had high operational proficiency. The students majoring in the molecular medicine had better prior knowledge of the techniques: there were only 9 lessons in which more than 25% of the students were not familiar with the theoretical knowledge, whereas there were 12 lessons in which less than 10% of the students had high operational proficiency. The four lessons with worst prior knowledge were cell proliferation, flow cytometry, image acquisition, and omics analyses. We grouped the 15 lessons into 7 categories: the ophthalmic experimental data collection, the omics technology, the data analyses, the cell technology, the protein technology, the tissue analyses, and the molecular technology, presenting by the level of the students’ pre-training proficiency from low to high. Conclusion: The first-year graduate students were very weak in basic knowledge and had little experience in techniques before joining the molecular medicine training courses. It is urgent to improve the laboratory skills training system and strengthen the construction of the curriculum system.

干眼是一种临床常见慢性眼表疾病。它不仅能引发患者眼部不适及损害,还易导致患者出现心理障碍。目前最常见的干眼相关心理障碍主要为焦虑、抑郁和睡眠障碍。干眼病情的缓解,不仅需要针对干眼本身的积极治疗,还需要患者保持健康的心理状态。因此,应该关注干眼相关焦虑、抑郁等心理障碍的发病机制及预防、治疗措施。
Dry eye disease (DED) is a common chronic ocular surface disease. It can not only cause discomfort and damage to the eyes but also easily lead to psychological disorders in patients. The most common psychological disorders related to DED are anxiety, depression and sleep disorder. To alleviate the condition of DED, in addition to active treatment for DED itself, it is also necessary for the patients to maintain a healthy psychological state. Therefore, attention should be paid to the pathogenesis, prevention and treatment measures of DED-related anxiety, depression and other psychological disorders.

药物治疗单纯疱疹病毒基质型角膜炎的研究进展

单纯疱疹病毒基质型角膜炎是引起角膜盲的主要原因之一,目前以局部使用糖皮质激素联合口服抗病毒药物治疗为主。传统治疗存在生物利用度低、药物不良反应等缺点,因此亟需寻找替代药物、开发新剂型。环孢素A和他克莫司等免疫抑制剂疗效明显、不良反应少,可能是糖皮质激素的潜在替代品。α干扰素联合阿昔洛韦可缩短病程,而单独使用效果有限。基质再生剂具有新的抗病毒机制,值得进一步研究。此外,纳米载体递送系统,如脂质体、纳米胶束、立方液晶纳米粒,由于能够增强药物角膜穿透性和延长药物释放,在治疗基质型单纯疱疹性角膜炎方面具有巨大潜力。
Herpes simplex virus stromal keratitis is one of the leading causes of corneal blindness. A topical corticosteroid
agent in conjunction with an oral antiviral agent is the preferred treatment, which has the disadvantages of low bioavailability and drug side effects. Therefore, there is an urgent need to find alternative drugs and develop new dosage forms. Immunosuppressants such as cyclosporine A and tacrolimus have obvious curative effects and few side effects, and may be potential substitutes for glucocorticoids. Interferon-α combined with acyclovir can shorten the course of disease, but the effect is not obvious when used alone. Matrix regenerating agents have new antiviral mechanisms and deserve further study. In addition, nanocarriers delivery systems, such as liposomes, nanomicelles and cubosomes, have great potential in the treatment of herpes simplex virus stromal keratitis due to their ability to enhance drug corneal penetration and prolong drug release.

息肉状脉络膜血管病变继发玻璃体积血的治疗及预后

Treatment modalities and visual prognosis of polypoidal choroidal vasculopathy with breakthrough vitreous hemorrhage

息肉状脉络膜血管病变(polypoidal choroidal vasculopathy,PCV)是亚洲人中常见的眼底致盲性疾病,当PCV合并视网膜下出血或玻璃体积血(vitreous hemorrhage,VH)时,患者视力骤然下降,视力预后差异大。但目前聚焦于PCV合并VH的相关文献较少,因此研究和阐明PCV继发VH的治疗方法及预后具有重要的临床意义。目前临床上常选择手术干预,玻璃体切除术(pars plana vitrectomy,PPV)是临床上最常选择的一种术式。其他治疗方式包括玻璃体内注射抗血管内皮生长因子(vascular endothelial growth factor,VEGF)、眼内气体或硅油填充、眼内注射组织纤溶酶原激活剂(tissue plasminogen activator,tPA)和光动力疗法(photodynamic therapy,PDT)。PCV合并VH患者的视力预后决定因素是黄斑视功能的保留程度,也与年龄、术前视力、PCV病变部位、视网膜下出血量、视网膜脱离范围、基线黄斑中心厚度(central macular thickness,CMT)、是否出现术后并发症以及是否形成视网膜瘢痕等因素相关,目前也有研究发现视力预后与单核苷酸多态性(single nucleotide polymorphisms,SNP)相关。本文就PCV继发VH的临床特点、治疗及预后进行综述。
Polypoid choroidal vasculopathy (PCV) is a common fundus blinding disease in Asians. When PCV is associated with subretinal hemorrhage or vitreous hemorrhage (VH), patient's visual acuity decreases suddenly and the visual prognosis varies greatly. There are few relevant literatures focusing on VH secondary to PCV, so it is of great clinical significance to study and clarify the treatment methods and prognosis of VH secondary to PCV. At present, surgical intervention is often selected in clinical practice. Vitrectomy is the most commonly selected surgical procedure in clinical practice. The other treatment modalities include intravitreal injection of antivascular endothelial growth factor (VEGF), intraocular gas or silicone oil filling, intraocular injection of tissue plasminogen activator (tPA) and photodynamic therapy. The prognostic determinant of visual acuity in PCV
patients with VH is the degree of preservation of macular visual function. The prognostic is also related to age, preoperative visual acuity, PCV lesion location, amount of subretinal hemorrhage, extent of retinal detachment, baseline central macular thickness (CMT), postoperative complications and retinal scars. Recent studies also find that the prognosis of visual acuity is related to single nucleotide polymorphisms. This article reviews the clinical characteristics, treatment and visual prognosis of PCV associated with VH.

猫抓病(cat scratch disease,CSD)是由巴尔通体引起的一种人畜共患病。该病不仅有多种全身表现,还可能出现各种危害视力的眼部并发症。随着家庭饲养宠物不断增多,CSD发病率逐年上升,眼科医生应重视此病。CSD的临床表现多种多样,容易误诊,与猫等宠物接触的病史、高滴度的血清免疫球蛋白G抗体是诊断的关键,聚合酶链反应也有助于诊断。由于CSD通常是免疫能力强的个体的自限性感染,因此通常不需要抗生素治疗。然而,当免疫力低或感染重时,多西环素是最常用的抗生素。
Cat scratch disease (CSD) is a zoonotic disease caused by Bartonella, which not only has a variety of systemic manifestations, but also may have various ocular complications that endanger vision. With the increasing number of pets kept at home, its incidence shows an increasing trend year by year. Therefore, ophthalmologists should pay attention to this disease. The clinical manifestations of CSD are various, which easily lead to misdiagnosis. The medical history of contact with cats and other pets and serum immunoglobulin G antibody with high titer are the key to diagnosis, and polymerase chain reaction is also helpful to diagnosis. Because CSD is usually a self-limiting infection of individuals with strong immune ability, antibiotic treatment is usually not required. However, when immunity is low or infection is severe, doxycycline is the most commonly used antibiotic.

眼缺血综合征(ocular ischemia syndrome,OIS)是由一系列诱因引发的以慢性眼部低灌注为主要临床表现的眼部疾病。临床相对少见,但对视力的影响较大,其症状主要包括一过性黑朦、缺血性眼痛、永久性视力丧失等。近年来医疗美容行业逐步兴起,自体脂肪填充技术使用逐渐增多,其所引起的OIS不可忽视。本文分析1例自体脂肪填充患者术后致OIS病例,研究该类疾病眼底影像学特征。
Ocular ischemia syndrome (OIS), featuring as chronic ocular hypoperfusion, is an eye disease caused by a series of incentives. It is relatively rare in clinical practice, but has a great impact on vision. The symptoms of OIS mainly include transient amaurosis, ischemic eye pain, permanent vision loss, etc. In recent years, with the rise of the Aesthetic Medicine industry, the technology of autologous fat filling has been increasingly adopted. The OIS caused by the surgery of autologous fat filling is nonnegligible. In this paper, by means of analyzing a case that an autologous fat filling surgery resulted in the OIS, discusses features of fundus angiography of OIS.

永存胚胎血管(persistent fetal vasculature,PFV),也称永存原始玻璃体增生症( persistent hyperplastic primary vitreous,PHPV),是一种先天性眼病,多发现于婴幼儿时期。因大多数患儿单眼发病、症状隐匿,且易被误诊为单纯的先天性白内障,常常延误手术治疗的最佳时机。因此,正确的诊断和适宜治疗方式的选择对于患儿视功能的预后尤为重要。本文报道了1例6岁6个月的男性患儿,诊断为混合型PFV,眼部特征表现为先天性白内障和黄斑区结构错位。
Persistent fetal vasculature (PFV), also known as persistent hyperplastic primary vitreous (PHPV), is a congenital ocular anomaly, which is common in infants and young children. Due to most children have unilateral occurrence, insidious symptoms and are easily misdiagnosed as simple congenital cataract, the optimum time for treatment is often delayed. Therefore, correct diagnosis and appropriate treatment are particularly significant for the prognosis of PFV children’s visual function. A male child aged 6 years and 6 months with a diagnosis of combined PFV is reported, whose ocular features were congenital cataract and structural dislocation of macula.

咽旁间隙肿瘤切除术后并发霍纳综合征1例

    霍纳综合征是一种由眼交感神经传出通路中任意部分中断所造成的临床综合征,常以上睑下垂、瞳孔缩小等眼部特征为主要临床表现,往往提示头、颈或胸部严重疾病或术后并发症的发生。现报告1例49岁的男性患者,曾在武汉市中心医院耳鼻喉科行右侧咽旁间隙肿物切除术,术后第1天出现右眼上睑下垂,瞳孔缩小,伴同侧面部无汗。术后随访半年,上述症状未见明显改善。右侧咽旁间隙肿物的病理结果证实为交感干神经鞘瘤,该病例临床较为罕见。
Horner syndrome is caused by damage of the oculosympathetic pathway. It is a common disorder characterized by ocular signs such as ptosis and miosis, and these signs usually indicate the occurrence of severe head, neck and chest diseases or surgical complications. We report a case of 49-year-old male patient who underwent parapharyngeal space tumor resection in the Department of otolaryngology. On postoperative day one, the patient presented right eyelid drooping, the right pupil constricted, and the absence of sweating on the right side of the face.  After six months of follow-up, the above signs still presented and showed no significant change. T??he pathological assessment of the resected parapharyngeal space tumor demonstrated that it is the sympathetic trunk schwannoma, which is relatively rare in clinical practice.
